Replacing Lost Certificates
Has your security certificate been lost, stolen or destroyed? We can issue a replacement.
You will need to send us a letter to advise that your certificate has been lost, stolen or destroyed (Please note: We need a letter by mail or by fax; an e-mail or other electronic message is not sufficient).
We will then place a stop against the certificate and provide you with information on the next steps (requirements vary from issuer to issuer).
Generally you need to provide:
- Proof by way of an affidavit that the certificate is lost.
- A surety bond to protect the issuer and CIBC Mellon if the original certificate is negotiated.
There are two types of surety bonds:
- One that is issued in response to an application from a security holder to cover a single instance of securities being lost, stolen or destroyed.
- A special surety bond that is maintained by or on behalf of an issuer to cover all that issuer’s security holders.
Issuers will accept individually issued surety bonds from acceptable surety companies, but using the special bond is more convenient for the security holder.
Many of CIBC Mellon’s clients use a special surety bond. To check if the issuer of the lost, stolen or destroyed securities belonging to you uses the special bond, check the issuer list on this website.
If your issuer uses the special bond, you can download a copy of the form we require.
Please do not start completing the form until you have received confirmation from us that the certificate is still outstanding and advising you how much the premium will be for coverage under the surety bond.
If the issuer does not use the special bond, you will need to follow the instructions we will send you, to obtain your own surety bond.
