Investor meeting planner
CIBC Mellon’s investor meeting planner can help plan your annual meeting. The planner incorporates requirements of National Instrument 54-101 (Communication with Beneficial Owners of Securities of a Reporting Issuer), and identifies all the information CIBC Mellon requires to perform our responsibilities in mailing materials, tabulating results, and scrutineering your meeting.
In addition to communicating with your registered investors, under NI 54-101 you may either request CIBC Mellon to mail to your Non Objecting Beneficial Owner (NOBOs) and tabulate their voting instructions, or have financial intermediaries perform this function, at your expense.
To complete the meeting planner electronically, please follow these instructions:

Download the meeting planner form, and enter details about your annual meeting. Input your date in the dd/mmm/yy format (e.g., 01/May/07). Please note dates must be business days and not weekends or holidays.

For help with any field, please move the cursor to that field and press the F1 key on your computer.

Save the completed form to a folder on your hard drive.

E-mail the completed form to TCS-Client_Meetings@cibcmellon.com. Please make sure to send a copy to your CIBC Mellon client service manager. You may also fax this form to 1-800-354-0463.
If you are using both electronic distribution and regular mail to communicate with your investors, please indicate this in the appropriate space on the form.
We ask that you forward the completed form to us at least a month prior to your scheduled record date.
